If you are in Group 2 (people who scanned one or more fingers at a Sky Zone Trampoline Park located in Elmhurst, Aurora, or Joliet, Illinois), you will need to submit a Claim Form. The Notice you received contains a Claimant ID that you can use to submit the Claim Form electronically. You can also submit a Claim Form by mail.

To receive money from the Settlement, the Claim Form must be electronically submitted or postmarked on or before June 24, 2024.
